CHS INDEMNITY & LIABILITY WAIVER

Please complete this waiver before entering or making use of any facilities operated by Country House School, including HopHouse Indoor Trampoline Park, Country Swim School, and Country House School. Your intentional action in electronically signing this document confirms your understanding of, and agreement to, the terms and conditions outlined below.

The person signing this waiver affirms that they are a responsible adult acting on behalf of all individuals in their party, including any accompanying adults and/or minors. By signing, they confirm that they have the authority to accept these terms and conditions for the entire group and assume full responsibility for their party’s compliance, conduct, and safety while using the facilities operated by Country House School.

I acknowledge that participation in physical and recreational activities at any venue operated by Country House School - including HopHouse Indoor Trampoline Park, Country Swim School, and Country House School - carries inherent risks, including but not limited to falls, sprains, collisions, equipment-related injuries, swimming-related incidents, and general activity-related harm. I voluntarily assume these risks on behalf of myself and/or the listed participants.
I accept full responsibility for any injury or loss sustained during participation at any of these venues, and I voluntarily assume all risks associated with these activities.
I authorise Country House School staff to seek emergency medical treatment if necessary and understand that any and all costs incurred will be my responsibility.
I waive any and all claims against Country House School, its venues, owners, employees, and affiliates, except in cases of gross negligence or willful misconduct.
I agree to follow all posted safety guidelines and instructions given by staff at all times, across all venues.
I understand this waiver does not absolve Country House School or its affiliated venues from responsibility arising from gross negligence.
I give permission for emergency medical treatment to be administered to myself or members of my party if necessary.
I indemnify Country House School, its venues, and staff against any legal claims, costs, or damages resulting from participation at any of the facilities.

I consent to photos and/or videos being taken of myself and my party during our visit to any facility operated by Country House School (including HopHouse Indoor Trampoline Park, Country Swim School, and Country House School), and I grant permission for these images to be used for promotional or marketing purposes.
